Clicking or pain in the jaw joint area is a common reason for visits. The symptom may be related to the joint, the muscles, the teeth or nearby structures. It is important to establish the cause, not just relieve the pain. Below is a step-by-step breakdown.

Where the jaw joint is located

The temporomandibular joint connects the lower jaw to the temporal bone. It is located in front of the ear. Joint movements enable opening the mouth, chewing and speaking.

There is a joint disc between the bones. It cushions the load. Disc displacement is often accompanied by clicking.

The joint works together with the chewing muscles. That is why muscle problems often produce similar symptoms.

Why the jaw clicks

Clicking is a sound that occurs during joint movement. The most common causes are:

  • displacement of the joint disc;
  • muscle dysfunction;
  • arthrosis of the joint;
  • overload from bruxism;
  • jaw injury.

Clicking without pain does not always require treatment. The decision is made after an examination.

The sound may occur only when opening the mouth wide or during chewing. The doctor clarifies exactly when it appears.

Sometimes clicking accompanies disc displacement without inflammation. In that case monitoring and habit correction are enough.

Causes of jaw joint pain

  • dysfunction of the chewing muscles;
  • inflammation or arthrosis of the joint;
  • displacement of the joint disc;
  • bruxism and night-time teeth clenching;
  • chewing on one side;
  • stress and chronic tension.

The pain may radiate to the ear, temple or neck. So the doctor rules out other sources of pain.

Tension in the neck and shoulders sometimes accompanies jaw pain. The doctor clarifies whether these symptoms are connected.

Pain after prolonged chewing or yawning more often indicates muscle dysfunction. This is an important detail for the diagnosis.

How to tell a joint problem from toothache

Toothache is more often related to a specific tooth. It increases with temperature or biting. Joint pain is usually related to jaw movement.

The doctor also rules out other conditions:

  • pulpitis and periodontitis;
  • trigeminal neuralgia;
  • diseases of the ear, throat and nose;
  • inflammation of the mucosa and gums.

Self-diagnosis is often wrong. An examination and, when indicated, X-rays are needed.

Sometimes a consultation with an ENT doctor or neurologist is needed. This is a normal part of diagnosis, not a sign of a complex case.

When urgent help is needed

  • the jaw is locked open or closed;
  • swelling of the face;
  • fever;
  • jaw injury;
  • severe pain that does not pass.

In these cases do not postpone the visit. Call the clinic.

If the mouth does not open or close fully, do not try to fix it yourself. This can make the condition worse.

How diagnosis is carried out

The doctor asks about the symptoms, examines the oral cavity, palpates the muscles and the joint, measures mouth opening and analyses the occlusion.

It is important to distinguish joint pain from muscle pain. The treatment plan depends on this.

It helps to write down when the pain appears and what makes it worse. Such a diary saves time at the appointment.

The doctor checks for limited movement and whether mouth opening is symmetrical. These are basic clinical reference points.

When indicated, a CT scan is prescribed. It shows the condition of the bone structures and the joint heads.

Treatment: from a splint to orthodontics

The plan depends on the cause. The goal is to reduce pain and load and to restore function.

Treatment starts with the least invasive methods. If they do not help, the next steps are discussed.

  • an occlusal splint to relieve the joint - bruxism splint;
  • occlusion correction when indicated;
  • orthodontic treatment if the cause is related to the bite - orthodontist and braces;
  • monitoring and follow-up examinations;
  • referral to related specialists when needed.

Surgical treatment is considered only in selected cases after a consultation. We do not promise complete disappearance of symptoms.

After treatment starts, the doctor assesses the dynamics. If symptoms do not decrease, the plan is reviewed.

Timelines depend on the cause and the patient discipline. A splint works only when worn as prescribed.

Connection with bruxism

Night-time teeth clenching creates additional load on the joint. A patient may not know about bruxism until wear or morning pain appears.

If bruxism is confirmed, joint treatment is combined with tooth protection. Details are in the article about bruxism.

What does not help and can cause harm

  • constant chewing on one side;
  • self “manipulation” of the jaw;
  • heat compresses during acute swelling;
  • prolonged self-treatment with painkillers without examination.

These actions do not remove the cause. In some cases they increase the pain.

Painkillers can temporarily hide the symptom. They do not replace diagnosis and do not treat the joint.

Pay attention to habits: chin position, resting on your hand, prolonged gum chewing. The doctor assesses these factors together with the clinical picture.

What to do before the visit

  • do not load the jaw with hard food;
  • avoid opening the mouth wide;
  • apply cold in case of swelling;
  • do not massage yourself without a doctor recommendation;
  • take a painkiller as directed.

Do not apply heat during acute swelling. It can increase inflammation.

Eat soft food in small portions. Avoid chewing gum and hard nuts before the consultation.

Do not open your mouth abruptly when yawning. Support your chin with your hand to limit the range.

Sleeping with your head raised sometimes reduces morning discomfort. This is a supportive measure, not treatment.

If the pain increases, do not wait for the scheduled time. Call and describe the changes.

Do not have an X-ray elsewhere before the visit without a recommendation. The doctor will determine the needed examination format.

In most cases treatment starts with conservative methods: a splint, occlusion correction, orthodontics, monitoring. Surgery is considered separately.

Orthodontist

Sometimes orthodontic treatment affects the bite and the load on the joint. The condition is assessed before and during treatment.

Can jaw pain radiate to the ear?

Yes, pain can radiate to the ear, temple or neck. So the doctor rules out diseases of the ear, throat and nose.

Symptoms may increase and the load on the teeth and joint may grow. The exact prognosis is determined after diagnosis. In some cases the condition remains stable, so monitoring is needed.
